Applicant certifies the information provided is true and authorizes credit investigation and contact with references and financial institutions. Invoices are due according to approved terms. Applicant agrees to pay all collection costs, reasonable attorney's fees, and applicable interest on past-due balances where permitted by law.

Personal Guaranty

In consideration of extending credit to the Applicant, the undersigned Guarantor unconditionally and personally guarantees the full and prompt payment of all present and future obligations of the Applicant. This guaranty is continuing, absolute, and unconditional and shall remain in effect until revoked in writing and accepted by the Creditor, provided such revocation shall not affect obligations incurred before acceptance. Guarantor agrees to pay all collection costs and reasonable attorney's fees where permitted by law.
